I need an outlet by a wall phone to plug a AC power supply for the phones speed dial memory. A light switch is right there, so I replaced it with a switch / outlet combination. My problem is that the power supply prongs dictate that it is plugged in right over the switch. If I could rotate the block from the power supply 180 degrees, it would fit, but I'd have to grind down the prong to fit in the recepticle. What's the best solution?

Are you sure you have a working receptacle. A lot of switches have no white grounded leg [aka white neutral]. Many switches only have a single Romex cable going from the light to that switch being a black hot wire and a white hot wire but no white neutral wire. Be sure you have a power source with a grounded white wire in that box.
Do not grind off the prongs of a plug. BAD IDEA, not safe !!
If you have to make an adjustment then by an adapter that has three receptacle plugins. Then plug that adapter into your receptacle and this will create a horizontal plug to plug your transformer into.

Most of the wall power packs I have seen are double insulated and don't have polarizing plugs. Look at the rating aon the pack and replace it with one that is not polarized. Radio shack has a variety of different types of theses wall warts.
