All my measurements are ttp, or to the top, because I hate having to stoop over to see if my mark is lined up with the bottom of a box 16" off the floor.
Also forget "hammer height". This is bogus even if workers say they "synchronize" hammers.
OK.
Standard receptacles - 16"
Standard switches - 48"
Kitchen counter rec. - 48"
Bath rec. - 48" or 44"
Dryer and washer rec. - 36"
Thermostat - 60"
Door chime - ~84"
Door bell button - 48"
Boiler switch - 60"

I think that 16" to the top of a receptical box is a little low to make things comfortable. (not comfortable to wire up and not comfortable to use) I like 16" to the bottom of the box. I'm on a job now where the mounting heights fall under Americans with Disabilities Act requirements. The receptacals are at 24" to the bottom of the box, and the switches are at 42" to the bottom of the box. I've noticed that some of the bretheren who wire tract homes mount the receptacles at 24" and the switches at 46", since they can scoot around on their little wheeled stools at finish time with higher productivity.

I know it's a locality thing. Most every one I know uses 16" to either the top or bottom. Mostly TTP. In fact some use 12" TTB.
