About one in every fifteen times I flush my toilet, the flap does not close, thereby causing the toilet to run continuously. I have tried adjusting the slack in the chain connected to the flap and replacing the flap. This has not solved the problem. Any suggestions?

What is happening is someone is flushing hard enough on the lever to make the flapper flip on over and hand against your overflow tube.

Easiest way and I know it sounds crazy but try it you might like it is to take a hose clamp and clamp the clamp onto the overflow drain pipe with the head of the clamp pointed toward where your flapper comes up. This may stop the flapper from flipping on over and hanging.

If you want to do it properly replace the entire flush system as a pakage replacing the drain pipe and flapper etc. This too should solve the stuck flapper problem.
