I need to move a cleanout as part of the sewer work for an addition we are building. The sewer pipe needs to make a 90 deg turn out the back of the house. I would like to make this bend using a 1/8 bend, then a flat wye. I would then attach a short horizontal extension piece to the wye and then a vertical extension for the cleanout (see attached picture). I read that cleanout should have a takeoff above the flow line, except for wye's. Does my configuration meet code with the flat wye?

Thanks,
David

Yes, you're fine. You need one cleanout for every change of direction greater than 45 degrees. The cleanout with the 22 degree bend into the wye satisfies the cleanout requirement for that 90 degree change in direction. The vertical cleanout at grade level is just icing on the cake, beyond code. It is normal to make the 90 degree change in direction at the bottom of the vertical cleanout with two 45's spaces a foot or two apart rather than with one 90.
