Forgot one question. Should I have the fabricator cut the seam for the countertop as a mitered corner or a straight butt joint. Is one type of joint better than the other. Thanks.

Your best bet is to thumb through the How-to books at your home center and spend the 20 bucks on one that goes into the most detail and has lots of good photos of the installation progression.
As far as mitre on the joints, they are usually cut straight. To get a real tight joint, there are clamps that attach to the underside of the counter at the joints that are a screw type. These screw clamps, by turning, draw the joint tight together. Trick is not to over tighten as this could cause buckling or worse, the lam to chip on the edge.
I'll add, my experience with counter top install is very basic and you want to look for others to offer their more expert advice.
