We a lot more detail. Probably best to have it inspected by a local septic person.

Unfortunately, your septic tank is in a bad way.

Septic tanks produce gases which include Hydrogen Sulfide which destroys concrete over time. This has happened to your tank. The inside of the blocks has been competely eaten away and the cracks go all the way to the top and continue accross it.

Your tank is now in a dangerous condition, and having once had a tank top collapse under me (luckily the tank contents were solid with sludge build-up!) I would not want to stand on yours.

You may be able to get a liner fabricated, but it would cost as much as a new septic tank. I think that it is time for a complete replacement, preferably with a plastic tank that is not affected by hydrogen sulfide gas.
