replacing park model roof

Has anybody replaced the roof of an RV. I own a 1995, 39 foot park model. There are some spots on the roof that are kind of soft. I think the actual plywood under the rubber roof is soft. I would like to replace it, including the roof sheathing, but do not know how to go about it. Any ideas or resources would be helpful.
